Online Survey Form

We are pleased to offer you the ThinkFirst Annual Chapter Survey for the 2007-2008 school year for completion online. Our goal is to receive surveys from 100% of our chapters in order to develop a complete and accurate snapshot of ThinkFirst!

The Annual Survey will:

1. Highlight details about local chapters to allow the ThinkFirst Foundation to better serve chapters in the future.
2. Assist chapters in evaluating their progress and achievements at the end of each school year.
3. Measure compliance with chapter requirements
4. Enable the Foundation to track important data to share with potential donors and media, as well as chapters and their organizations, helping them recognize the impact of our programs.
5. Update any contact changes for chapters and sponsoring physicians

Please keep in mind the following information while completing the survey:

• This survey pertains to your chapter's activities during the 2007-2008 school year.
• If your local chapter is not active and does not plan to become active in the near future, please indicate this in the space provided. It is important that we have accurate information regarding active chapters, as we field inquiries on chapter locations
• Please submit no later than July 31, 2008. This survey will be available for only a short time after this date, so please act now!

Instructions

All ThinkFirst chapters must submit the 2008 Annual Survey by July 31, 2008. Our goal is 100% compliance! Your complete information is crucial in our ability to report determine and accurate information.

1. Please click on the link below to access the ThinkFirst Annual Survey.
2. Enter the password "2008survey."
3. Read the directions at the beginning and begin entering your answers.
4. Each question marked with an asterisk (*) must be completed before you are allowed to advance to the next page.
5. Your answers are not saved on a page until you select "next." You may then choose "Previous" without loosing any data.
6. There is not an option at the end to save a full copy or print the entire document. To do so, you will need to save or print each individual page.
7. You do not have to complete the survey all at one time: you may choose to exit survey at the completion of any page. As long as you are using the same computer, click on the link and enter the password "2008survey" to continue. (Be sure to select "next" on your last completed page to save your work first!)
8. Once you select "done' at the end, a page will thank you; your next click returns you to www.thinkfirst.org. Your survey has been submitted!
